Our Services

24-Hour Emergency Assistance


Community Provider Home Services Minnesota  offers on call counseling and problem solving and/or immediate response for assistance at an individual’s home due to a health or personal emergency.

Management and DSP’s are available around the clock to address your personal needs and concerns. This service is ideal for individuals that live independently but need reassurance that in case of emergencies that there is someone available to them.


Adult Companion Services


Adult Companion Services may assist or supervise the person with tasks such as

  • laundry,
  • light housekeeping,
  • meal preparation
  • shopping only while he or she helps the person work toward a therapeutic or community integration goal.
Home Maker Services

Home Maker Services that help a person manage general cleaning and household activities.

  • Homemaker/home management

  • Arranging for transportation
  • Laundry
  • Meal preparation
  • Shopping for food, clothing and household supplies
  • Simple household repairs.
Personal Support

Services provided in the person’s home or in the community to:

  • Achieve his/her full potential
  • Increase his/her independence
  • Meet community inclusion goals that are important to and important for the person and based on an assessed need.

Personal support services relate to outcomes identified in the person’s support plan when teaching and training is not necessary to achieve these outcomes, and it is the least costly assistance to reasonably meet the person’s needs. Covered services include supervision, support or assistance with:

Respite Care Services

Respite (In Home or Out of Home)

  • Respite is a short-term service when you cannot care for yourself and your primary live-in caregiver needs relief.
  • Traditionally, we care for people with disabilities in their own homes for periods of 12 hours or more. For example, we might begin care on a Friday evening and continue through the weekend to Sunday

Crisis Respite

  • Respite Short-term care services provided to a person when his/her primary caregiver is absent or needs relief

Independent Living Skills Training (ILS)

In home family support is provided to adults with developmental disabilities.

The services are designed to enable each individual the opportunity to remain in or return to their home. This includes training of the person and family members to increase their capabilities to care for and maintain the person in the home. Staff work on skills that will lead individuals to greater independence and self-sufficiency some of the goals worked on are below.

  • Household chores (e.g., bed-making, washing dishes, laundry)
  • Eating and food preparation
  • Personal hygiene and cleanliness
  • Social and adaptive skills necessary to help the person live in the community
  • Health, safety and wellness
  • Household management.
  • Exercise
Individual Community Living Support (ICLS)

Independent Living Skills (ILS) are services that develop, maintain and improve the community-living skills of a person.

  • Communication skills
  • Community living and mobility
  • Interpersonal skills
  • Reduction/elimination of maladaptive behavior
  • Self-care
  • Sensory/motor development involved in acquiring functional skills



Residential Habilitation

Services provided to a person who needs support to live in the community. The services are provided in the person’s own home and community, and they are directed at increasing and maintaining the person’s physical, intellectual, emotional and social functioning. Residential habilitation includes assistance to develop, maintain or improve skills related to activities of daily living (ADLs).

In home family support is provided to adults with developmental disabilities.

Supported Living Services (SLS)

  • Services provided to a person who requires daily staff support for challenging behaviors, medical needs, physical needs and/or life skills.
  • For people younger than age 18, SLS is provided to children who live outside of the biological or adoptive home.
